This is the building on the NE corner of 12th and Division St.  The Artemis Café is at the front of the building,  The office is inside the complex, first floor, #115. (We are NOT in the craftsman house across the street which also houses an acupuncturist and chiropractor.)

Parking: Street parking is available and usually easy to find. There is no parking lot for this building.

I am a recognized provider with Regence Blue Cross Blue Shield of OR, The ODS Company, Providence, Lifewise of OR, Cigna, HealthNet and American Specialty Health.

To identify me specifically, my National Provider Number (NPI) is: 1366624413.

If your insurance plan covers acupuncture from practitioners out-of-network, we would be happy to bill your insurance. We do this often with Aetna, OHP, United and GreatWest to name a few.

In order to facilitate this process it would be helpful for you to call your insurance carrier in advance to determine the specifics of what your plan covers. You can download a PDF List of Questions to help in attaining the necessary information from your insurance carrier.

 

• Arrive at your initial appointment early to complete paperwork or with downloadable forms completed.
• Wear loose fitting comfortable clothing that allows access to just above the elbow and the knee.
• Come with some food in your system, but ideally not immediately after a large meal. Acupuncture on an empty stomach can cause some people to feel light-headed, sweaty and/or nauseous.
• Please arrive on time, or even better, 5 minutes early, so you can start to relax into YOUR treatment time.

Cancellation policy: I ask my patients to recognize that scheduling an appointment involves the reservation of time specifically for them, and that consequently, a minimum of 24 hours notice is required to reschedule or cancel an appointment. Unless otherwise agreed to in advance, a partial fee may be charged for sessions missed without such advance notification. Insurance companies do not reimburse for missed sessions. Thank you for your attention and consideration of this matter.
